Good filler work is the kind no one can identify. It looks like you at your best, not like you have had something done. The skill is in understanding what your face actually needs, and using considerably less of it than you probably think you want.

What are dermal fillers?

Dermal fillers are injectable gels used to restore lost volume, soften lines, define contours, or enhance features like the lips and jawline. The most commonly used fillers are made from hyaluronic acid (HA), a substance naturally found in the skin that attracts and retains water.

At Skin Trek, we use Restylane, Juvederm, and Saypha. These are three of the most well-studied HA filler brands globally, each with a range of products formulated for different tissue depths, treatment areas, and desired outcomes. The choice of product matters, but it matters less than where it is placed, how much is used, and whether the plan is appropriate for your anatomy in the first place.

HA fillers are reversible. They can be dissolved with an enzyme called hyaluronidase if the result is not as expected, or if there is a complication. This is an important safety consideration, and one reason we prefer HA fillers for most areas over non-reversible alternatives.

What dermal fillers treat

Cheeks and midface

Volume loss in the cheeks is one of the most visible signs of facial aging. As subcutaneous fat descends and bone density decreases, the midface flattens and the nasolabial folds deepen. Strategic filler placement in the cheek area can restore projection and lift the mid and lower face without touching those folds directly. This is an area where less is almost always more.

Nasolabial folds and marionette lines

These lines run from the nose to the corners of the mouth (nasolabial folds) and from the mouth corners downward (marionette lines). Filler can soften them, but treating them in isolation often produces an unnatural, filled appearance. The better approach in most cases is addressing the midface volume loss that is causing them to appear, which softens these lines as a secondary effect.

Under-eye (tear trough)

The tear trough is one of the most technically demanding filler areas. Done well, it reduces hollowing and dark shadowing under the eyes significantly. Done poorly, it can cause persistent swelling, a bluish discoloration called the Tyndall effect, or an unnatural look. We assess candidacy carefully at consultation. Not everyone is a good candidate, and we will tell you honestly if you are not.

Lips

Lip filler is one of the most requested treatments and one of the most commonly overdone. At Skin Trek, the approach to lips is conservative by default. The goal is natural definition, hydration, and a small amount of volume, not a dramatic change in shape. We use small amounts and build gradually over multiple appointments if needed. A maximum of 0.5 to 1ml per session is our standard starting point for most patients.

Jawline and chin

Jawline definition has become one of the most popular filler applications in recent years, particularly among younger patients. Filler along the mandibular border can improve definition and create a cleaner jaw-to-neck transition. Chin projection can be enhanced subtly with a small amount of filler, improving facial proportions without surgery.

Temples

Temple hollowing is one of the most overlooked signs of facial aging, and one of the most impactful to address. Loss of volume in the temples creates a skeletal appearance, makes the face look tired, and can contribute to brow descent. A small amount of filler in this area produces a significant improvement in facial framing.

Hands and décolletage

HA fillers can also be used to restore volume in the backs of the hands, where prominent tendons and veins become more visible with age, and in the décolletage to soften crepey skin texture. These areas require different product choices and dilution strategies than facial applications.

Fillers vs. biostimulators

This is the most important conversation to have before committing to a filler treatment plan. HA fillers are appropriate when the goal is a specific shape change or targeted volume in a defined area. They are not the ideal tool for patients whose primary concern is diffuse structural volume loss across the face.

For patients who have lost significant volume in the cheeks, temples, and lower face over time, replacing that loss with multiple syringes of HA filler can produce an overfilled appearance that does not look natural because it is not replacing like with like. Biostimulators like Sculptra restore collagen, which is what was actually lost. The result integrates more naturally.

Many patients benefit from a combination of both approaches, with biostimulators for the foundational structural work and HA fillers for specific refinements. We will not use more filler than is appropriate for your face and your goals.

Fillers and East Asian anatomy

East Asian facial anatomy has specific characteristics that influence how filler treatments should be planned. The midface tends to have a flatter profile with less prominent cheekbones. The nasal bridge is typically lower and wider. The lips often have a different proportion between the upper and lower lip than is common in Western aesthetic ideals.

Understanding these differences matters practically. Filler techniques designed for Western facial proportions do not translate directly to East Asian faces. Applying the same cheek filler volumes and vectors, or the same lip ratios, produces results that can look unnatural on Asian anatomy. The aesthetic priorities and anatomical considerations are familiar, not theoretical.

One specific consideration: the risk of vascular occlusion, a serious but rare complication of filler injection, is potentially higher in certain anatomical areas in East Asian patients due to differences in arterial anatomy in the nose and glabella. We use ultrasound-guided filler dissolve as a safety service, and we apply careful aspiration and low-pressure injection technique in all high-risk areas.

Ultrasound guided filler dissolve

Skin Trek offers ultrasound-guided hyaluronidase treatment for patients who have had filler placed elsewhere and are experiencing complications, asymmetry, or results they are unhappy with. Using ultrasound imaging to visualise the filler before dissolving it allows for more precise, targeted treatment than blind injection of hyaluronidase, which can affect tissue beyond the intended area.

Frequently asked questions

How long do dermal fillers last?

This varies by product, area, and patient metabolism. Most HA fillers last between 9 and 18 months. Lips tend to metabolise filler faster, typically 6 to 12 months. Cheek and temple filler generally lasts longer. With repeated treatments over time, some patients find they need less frequent top-ups as residual filler accumulates in the tissue.

How much filler will I need?

Depends on what you are treating. A single syringe is appropriate for conservative lips or a subtle cheek enhancement. More comprehensive work may need two to four syringes across multiple areas. We give you an honest number at consultation. The honest number is usually smaller than whatever you found on Reddit.

Is there downtime?

Most patients experience mild swelling and occasional bruising for a few days. The lips tend to swell more than other areas and may look more pronounced than the final result for the first three to five days. We recommend avoiding significant social commitments in the 48 hours after lip filler in particular.

Can filler be dissolved if I do not like the result?

Yes. HA fillers can be dissolved with hyaluronidase at any time. At Skin Trek, we offer ultrasound-guided dissolve for precision. Dissolving filler is not instantaneous and may require more than one session, but it is effective and reliable.

Is filler safe for Asian skin?

Yes, with appropriate technique. The main considerations specific to Asian patients are anatomical, not related to skin type. Certain injection areas carry higher vascular risk in some Asian anatomical variants, which is why aspiration technique, low-pressure injection, and knowledge of ethnic anatomical differences are important. We take all of these precautions as standard.
